SOCIAL WORKER - MASTERS - MENTAL HEALTH OUTPATIENT (25-R559)

Status: TEMPORARY Full Time
Department: Mental Health Outpatient Program
Site: St. Joseph's Health Centre
Shifts: Days
Rate of Pay: $46.98 - $56.75 per hour
Reporting to the Clinical Manager, the Social Worker provides services to clients and/or their Substitute Decision Maker (SDM), family members, and other support persons in the client’s life when applicable. From herein, the job description will refer to clients and/or others for this purpose. The Social Worker is responsible for providing comprehensive assessment and intervention that addresses the psychosocial needs of the clients and/or others. The Social Worker is an integral part of the Interprofessional team, providing a social work perspective and acting as a liaison and/or consultant to internal and external programs within the scope of practice. Identifies opportunities for and participates in quality improvement activity within the care setting.
The Master of Social Work (MSW) level position requires autonomy, leadership and specialization in practice.
The goals of Social Work practice are to restore, maintain and enhance the quality of life by assessing, mobilizing strengths and coping capacities, providing treatments to modify patterns of relating, and acting, linking people to necessary resources, addressing environmental stressors individually/collectively and providing psychosocial education related to wellness and subjective well-being.
Qualifications:
- Masters of Social Work Degree and current registration with the OCSWSSW
- Knowledge and experience with referral processes and triage, decision guide processes and system navigation, assessing and treating individuals with serious and persistent mental illness
- Demonstrated skills in assessments, treatment, individual and group counselling
- Training and experience in motivational interviewing and solution focused methods, principles in self management and strengths-based interventions
- Experience/training in a variety of evidence based therapies such as: solution focused therapy, EMDR, CPT, CBT, DBT, and group therapies, ASIST training or equivalent
- Knowledge and skills in a range of theoretical perspectives, psychosocial rehabilitation principles, self-management and holistic wellness approaches
- Demonstrated ability to work as part of an interprofessional team with skills, knowledge and attitudes in: interprofessional communication; patient/client/family/community-centred care; role clarification; team functioning; collaborative leadership and interprofessional conflict resolution
- Knowledge of Indigenous worldviews, cultural practices and understanding of the impact of colonization on Indigenous populations an asset
- Excellent oral and written communication and interpersonal skills, judgment, critical thinking skills, problem-solving and time management skills
- Ability to utilize technology to effectively document in electronic medical records and retrieve relevant information
- Knowledge of the work and programs within St. Joseph's Care Group mental health and community resources, and an understanding of relevant legislation (i.e., Mental Health Act, Health Care Consent Act, PHIPA), knowledge of other community services and programs for adults
- Demonstrated regular attendance at work
CONDITION OF EMPLOYMENT
This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
Offers of employment made to external candidates are conditional upon providing proof of COVID-19 vaccination as per the or proof of valid medical exemption, prior to the offered effective date. If proof of vaccination or valid medical exemption is not provided, the offer of employment is rescinded.

Working to support adults, and their families, who have complex social needs and are more vulnerable as a result of disability, exclusion, or diminished capacity, you’ll maximise their potential to live independently. With case work focusing on complex adults recovering from a Stroke, you’ll ensure that adults and their families are safeguarded. You’ll share ideas and best practice, providing specialist advice as part of a multi-disciplinary team sharing your knowledge as a Social Worker. You will be supporting adults to leave the hospital, linking in with the Stroke Team and Early Supported Discharge therapists maximising the adults recovery and independence. We currently have an opportunity for a Social Worker, within our North Essex area. There will be the requirement to work weekends (one weekend in six) and bank holidays. Flexible working arrangements (depending upon service requirements including a proportion of time working from home supporting the core hours of 8am to 6:30pm) North Essex covers our offices in Colchester and Clacton. It is a diverse area to work in encompassing rural , costal and urban communities all with their own unique challenges. Essex County Council work closely with their partners in North Essex, those joining teams in this area are uniquely positioned to be increasingly involved in partnership projects across local authorities and the NHS.
