As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.What will you do? Provide administrative support services to Care Coordinators Process new referrals, and orders for services, supplies and equipment Process and assist in managing confidential patient records Enter, update and maintain a high volume of patient data in the electronic database Answer a high volume of telephone inquiries from patients, families and service providers, and refer callers as appropriateProvide back-up support to other positions, as requiredWhat must you have? A Grade 12 diploma (minimum) 2+ years’ related office experience Accurate keyboarding/data-entry skills Proficient with database software, MS Word, Excel, and other applications in a Windows environment Excellent organizational skills and ability to work independently with minimal supervision Advanced multi-tasking skills, with the ability to meet performance and service goals Exceptional interpersonal skills and ability to work as part of a team and interact tactfully and sensitively with patients from wide-ranging cultural, ethnic and socio-economic backgroundsExcellent oral and written communication skills, including strong listening skillsFlexibility to work a schedule that includes days, evenings and weekends to meet organizational needsWe have a mandatory COVID-19 vaccination policy. The role facilitates patient flow through the efficient allocation and utilization of beds, ensures adequate staffing levels and supports patients receiving quality care. The Patient Care Coordinator assists the Program Leaders in establishing and maintaining standards of patient care, and is responsible for coordinating the operations in the absence of managers, department heads and senior administrative personnel.Fort St. John Hospital and Health Centre has 44 Acute Care beds and 124 residential beds and is equipped to handle 22,000 ER visits per year. Some more highlights about the hospital:• Endoscopy suite, 2 state-of-the-art operating rooms, plus a procedure room• Ambulatory clinics including Cancer and Hemodialysis• Medical and Surgical inpatient rooms (40 beds)• 3 Intensive care units• Minor surgeries• 7 single Maternity rooms for Labour, Delivery, and Post-Partum Fort St. John also has the Health Unit and Primary Care Clinic which serve the community.Fort St. John is known as “The Energetic City,” which reflects the large resource base of oil, natural gas, forestry and agriculture. Job SummaryThe Patient Care Navigator provides personalized support to patients throughout their treatment journey, ensuring they have the resources needed to successfully start and stay on their medication regimen.Job Responsibilities:Assists in setting up new patients on specialty drug therapies by providing product information, obtaining prior authorization requirements, scheduling follow-up appointments for lab work or other services required for continued therapy, and addressing any adherence issuesMonitors all aspects of specialty pharmacy services for assigned patients including, but not limited to, refill requests, appointment scheduling, laboratory results, and billingCommunicates with providers regarding patient care needs, adverse events, medication side effects, and/or medication interactionsMaintains accurate documentation of all patient interactions in the electronic medical record systemProvides assistance to healthcare professionals related to medication use and disease state managementEducates patients on proper use of medications, potential side effects, drug interactions, and lifestyle modifications necessary to maximize therapeutic outcomesQualifications:Bilingual in English/French is an assetRegistered Nurse (RN) or Pharmacist (RPh) in good standingSuperior customer service skills with demonstrated ability to collaborate, and build strong relationships with patients, Physicians, and all members of the team.Self-directed and self-motivated with the ability to work independently while being part of a team.Acquires strong decision making and problem-solving skills with demonstrated ability to multi-task.Strong business acumen and ability to adapt to a corporate work environment.Excellent clinical assessment and clinical decision-making skillsExperience in reimbursement navigation, special authorization, appeals process and conducting field-based reimbursement support and consultationKnowledge of private and public reimbursement structure, systems and process.Strong analytical skills including interpretation of regulation and legislation.Previous experience using Patient Support Program Software/Technology is an asset.Able to travel when required, some overnight travel may be required.Ability to work in a dynamic, rapidly changing work environment.Excellent verbal and written communication skillsStrong organizational skills and attention to detailProficiency in computer applications such as Microsoft Office Suite and web-based software systemsWe are proud to offer a competitive compensation package at McKesson as part of our Total Rewards.
