Visit us today and check out to see why more people from across the globe are moving here.About the OpportunityThe Health Care Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the care continuum.  Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources. Working collaboratively with the healthcare team, the BSW creates a comprehensive plan for risk reduction and timely transition through assessment, planning, intervention, and evaluation. The Social Worker exhibits sound professional judgment and initiative while working within the Mission, Vision and Strategic Directions of NSHA. This position requires work at multiple sites and travel between various community locations throughout the area.  Successful candidates may be eligible for signing bonus incentive and relocation assistance with a defined Return of Service ContractAbout YouWe would love to hear from you if you have the following: Bachelor Degree in Social Work requiredMember of, or eligible for, membership with the Nova Scotia College Social Workers (NSCSW)Minimum of two (2) years' experience requiredValid driver's license and/or access to reliable transportation requiredAbility to organize and maintain client health records within and proficient computer skillsRecent, relevant experience working with community agencies/resources, volunteers and support groupsDemonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning), interviewing and client-centered problem solving, care planning, case management, evaluation of client competence in self-care and other health related outcomesDemonstrated ability to communicate effectively with patients, families, public, community partners, primary care team members and other professionals and agencies using verbal, written and computer communication meansExhibits characteristics of warmth, empathy, humor, and a non-judgmental philosophy of careDemonstrated critical thinking skills within scope of practice including clinical judgment and problem-solving skills and the ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectivelyAbility to promote client-focused care that demonstrates care for and with clients and significant others, sensitive to diverse cultures and preferences, client advocacy and social justice concerns strong understanding of health promotion, population health, and the determinants of health theory and practiceAbility to transmit information grounded in behavior change and principals of self-management intended to instruct clients and others about topics essential to health care and well-beingExperience in interdisciplinary and family meetings Manages time and resources, as well as collaboration across disciplines and related activitiesCompetencies in other languages an asset, French preferred Please ensure your resume is up to date and includes all relevant education, experience, training, and certifications.Hours of WorkPermanent full-time position; 75 hours biweeklyShifts may include evenings, weekends, and holidaysCompensation and Benefits$36.58 - $7.35 hourly Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan. Once You've AppliedThank you for your interest in this position.
